About Anthem Health Plans

A quick look at what Anthem is and how its plans work

Anthem Blue Cross Blue Shield is a subsidiary of Elevance Health. It provides in-state health insurance for people living in the following states:

Colorado
New Hampshire
Connecticut
Ohio
Maine
Georgia
Kentucky
Virginia
Wisconsin
Nevada
Indiana
Missouri

Affiliated Anthem companies also offer coverage in California and New York. About a third of Americans are insured through Anthem or one of its related plans, and its network includes more than 1.5 million providers nationwide.

Anthem covers a wide range of healthcare needs and offers multiple plan options for individuals and families. Some people buy coverage on their own, while others enroll through an employer.

Anthem HMOs

Many Anthem plans function as health maintenance organizations (HMOs). HMOs tend to feature low costs for monthly policy premiums. However, they typically offset this advantage by restricting your coverage to in-network providers outside of emergency situations. In addition, most HMOs only cover you for treatment obtained in your home state.

Anthem PPOs

In contrast, some Anthem plans function as preferred provider organizations (PPOs). On the downside, PPOs typically charge you more in monthly premiums. But on the upside, they give you more choices when it comes to using your desired treatment provider. It will cost you more to use an out-of-network PPO provider. Nevertheless, you will still get coverage for at least some of your expenses. Importantly, you don’t necessarily have to be in your state of residence to receive this coverage.

Using Anthem’s BlueCard Network

New Jersey isn’t one of the states where Anthem is based, but your plan may still help cover care here. That’s because Anthem uses a nationwide PPO network that allows members to use benefits outside their home state.

That network is called BlueCard PPO. If you have a Blue Cross Blue Shield plan, BlueCard lets you access participating providers across the broader BCBS system, even when the local plan uses a different name. The network is available in every U.S. state, as well as Puerto Rico and Washington, D.C., and includes most major hospitals and providers.

You don’t need to sign up separately to use BlueCard PPO. If your Anthem plan includes PPO benefits, you can use them right away. To make sure everything goes smoothly, here are a few steps to keep in mind:

Make sure the level of care you need is covered under your plan
Check whether that care requires preapproval
Confirm that the provider or facility is in the BlueCard PPO network
Bring your insurance ID card and point out the “PPO suitcase” symbol so the provider bills correctly

Anthem Coverage: What’s Typically Included

Anthem plans often include benefits for substance use treatment, but what’s covered can vary by policy. In general, coverage applies to care for substance use disorders, including:

Clinical addiction to medication, drugs or alcohol
Non-addicted substance abuse that impairs health, relationships, or daily life

Treatment for substance use is never one size fits all, so plans can look a little different from person to person. Still, Anthem or BlueCard PPO often helps cover the core services most people need, such as:

Substance detoxification (detox) under medical supervision
Psychotherapy or counseling
Medication
Combined medication and therapy, i.e., medication-assisted treatment (MAT)
Additional treatment for a mental health condition that occurs alongside an SUD

Most Anthem plans include outpatient care, and many also contribute toward inpatient or residential treatment when that level of support is needed. If your Anthem coverage uses the BlueCard PPO network, you may be able to use those benefits here in New Jersey as well.
