Please note that coverage varies based on individual provider, insurance plan in-network and out-of-network status, deductible, and services needed. Fill out the form or reach out to one of our admissions specialists and provide them with your insurance information. Once we receive this information, our admissions department can contact your insurance company to confirm your benefits, and then explain to you what your plan covers and how much you may pay out-of-pocket when treatment starts.

SOBA New Jersey’s Admissions Coordinators will walk you through the entire admissions process. They’ll help you verify your insurance, discuss your treatment options, connect you with additional resources, perform an over-the-phone assessment, schedule the start of treatment, and much more.
Travel Arrangements
SOBA NJ offers free travel to and from our facility for anyone within a 5-hour drive of our NJ facility. If you need assistance planning travel, we take care of everything for you so you can focus on the one thing that matters: your recovery.
